Updating Your Enrollment Forms

At the beginning of every year new "Parent Packets" are handed out to all of our Full-time, Part-time and Drop-in clients. Also if any personal information changes such as address, telephone or other relevant information, new forms must be updated. You can either ask me for a copy of these forms or you can download the forms fill them out and then simply bring the forms to the daycare and turn them in.

The above documents were designed to be used with Adobe Acrobat 7 and above. Depending on your version of Acrobat Reader your viewing experience may vary. Downloading a newer version of Acrobat Reader may be helpful.

When filling out your forms please be sure to:

  • Use only blue or black ink.
  • Sign, date and initial all of the appropriate sections of the forms.
  • Do not fill out the rates section of a new Contracted Schedule form, only your new schedule.
  • Do not cross out, make notes on or otherwise alter any portion of the Contract Agreement.
  • Use separate forms for each child.
  • Fill out and complete all sections of the Enrollment Forms.
  • Personally discuss all changes with your childcare provider.
  • Please note that changes to your Contracted Schedule will NOT be effective until the child care provider signs the new Contracted Schedule form.
